Why a Proper Investigation Matters More Than Most Businesses Realise
The instinct after a workplace accident is often to get things back to normal as quickly as possible. That is understandable, but moving on without a proper investigation leaves your business exposed in ways that are not always immediately obvious.
Without understanding the root cause of an incident, the underlying conditions that led to it remain unchanged. The next accident may involve a different person in a different location, but if the same systemic failure is present, it is only a matter of time. Businesses that treat investigation as a box-ticking exercise after the fact consistently see higher incident rates than those that treat it as a genuine learning process.
There is also a legal dimension that cannot be ignored. The HSE expects employers to investigate workplace accidents and take action on the findings. Insurers and loss adjusters will want to understand what happened and whether the employer took reasonable steps to prevent it. In cases where enforcement action or civil claims follow, the quality of your investigation and the actions taken as a result will be scrutinised closely.

What Workplace Accident Investigation Actually Involves
A workplace accident investigation is not just writing down what happened and moving on. It is a structured process that requires the right expertise, the right methodology and the right documentation to produce findings that are genuinely useful and legally defensible.
Our investigation process follows HSG245, the Health and Safety Executive's guide to investigating accidents and incidents. This is the recognised standard for workplace investigation in the UK and forms the basis of a thorough and professionally conducted inquiry.
The process begins with securing the scene and preserving evidence before it is disturbed, altered or lost. In the immediate aftermath of a serious incident this is critical, and getting a specialist involved quickly makes a significant difference to the quality of the investigation that follows.
From there we move through structured evidence gathering, witness interviews, physical inspection of the scene and any equipment involved, review of relevant documentation including risk assessments, training records and maintenance logs, and a detailed cause analysis that works back from what happened to identify the immediate causes, the underlying factors and the root causes that allowed the incident to occur.
The investigation concludes with a full written report setting out the findings, the cause analysis and a set of actionable recommendations prioritised by the severity of the risk they address.

Accident Investigation and Your RIDDOR Obligations
Certain categories of workplace accident must be reported to the HSE under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013, known as RIDDOR. These include fatalities, specified injuries such as fractures and amputations, injuries that result in more than seven consecutive days away from work, and dangerous occurrences that had the potential to cause serious harm even if no injury resulted.
Getting RIDDOR reporting right matters. Failing to report a notifiable incident is a criminal offence. Reporting inaccurately or incompletely can create further problems if the HSE decides to investigate. And the information you report needs to be consistent with your own investigation findings.
HSQE Consultancy can support you through the RIDDOR reporting process, making sure your obligations are met correctly and that the information reported accurately reflects the findings of your investigation. If the HSE decides to carry out its own investigation following a RIDDOR report, we can also help you understand what to expect and how to engage with that process constructively.

Our investigation reports set out a clear account of the incident, a detailed analysis of the immediate, underlying, and root causes, and a prioritised set of recommendations to prevent recurrence. Reports are written to a standard that satisfies regulatory scrutiny and can withstand review by the HSE, insurers, or legal representatives if required.
Timescales vary depending on the complexity of the incident, the number of parties involved, and the volume of evidence to be reviewed. Employers have a legal duty under the Management of Health and Safety at Work Regulations 1999 to review and update their risk assessments following incidents, which in practice requires investigation to understand what went wrong. RIDDOR additionally requires certain incidents to be reported to the HSE. Beyond legal compliance, thorough investigation is the most effective way to prevent the same incident from occurring again.
Yes, and in many cases these are the most valuable investigations to carry out. A near miss or dangerous occurrence that did not result in injury may have come very close to doing so. Investigating it properly and addressing the causes is how you prevent the same scenario from resulting in a serious injury the next time it occurs. RIDDOR also requires certain dangerous occurrences to be reported even where no injury resulted.
Yes. Once the investigation report has been delivered, we can support you with implementing the recommendations, reviewing the risk assessments and control measures that the investigation identified as inadequate, and communicating the findings and actions to your workforce. We can also carry out a follow-up review at an agreed point to assess whether the recommended changes have been implemented effectively.
Yes. Incidents involving contractors, visitors or members of the public on your premises can be investigated in the same way as incidents involving your own employees. Where multiple parties are involved, the investigation needs to be clear about the responsibilities of each party and how those responsibilities relate to the causes identified.
